Intensive Outpatient Services are available through the Adventure Program and school-based services are available at locations throughout the region. Frontier’s hard-working Case Management team supports treatment by helping families and children figure out the services they need and empowering them through person-centered planning.

A Story of Hope

These are the true stories of hope about real people from within Frontier Health. We have changed the names to protect their privacy.

“Tommy” suffered from severe abuse and neglect and found hope with Frontier Health’s Early Childhood Case Management Program. He was only 12 months old when abuse and neglect finally put him in the hospital where he was treated for severe burns and septic shock.

At 22 months old, his custodial family and his Frontier Health case management team began working together to complete his developmental screenings along with the educational activities “Tommy” needed to support his future development. Without this service so many children just like “Tommy” would fall through the cracks.

Your gift is the reason this family is flourishing, and “Tommy” has a real chance for a full and happy life.

Through your continued dedication, you have made an enormous impact and have helped Frontier Health improve thousands of children’s lives throughout Northeast TN and Southwest VA. We invite you to be a part of our growing community of givers and help us continue this great work!
